Abstract

Портал гипертензия - жигар касаллигининг асоратидир, бу жигарнинг портал қон оқими тизимидаги қаршиликнинг ўзгариши билан тавсифланади. Портал гипертензиянинг кучайиши натижасида томир коллатераллари шаклланиб, вазодилатациянинг ривожланиши кучаймоқда. Жигар циррозининг ривожланиши натижасида гипердинамик кўринишлар автоном нейропатия билан бирга шунтланишнинг кўпайишига олиб келади. Жигар етишмовчилиги ва портал гипертензия комбинациясида бир вақтнинг ўзида спланхик ва периферик артериал ўзаннинг вазодилатацияси ҳисобига қон айланишининг гипердинамик тури ривожланади. Гипердинамик синдром жигар циррозида портал гипертензиянинг кечки асоратидир. Жигар циррозини ўрганишда гипердинамик синдромнинг асосий белгилари юқори юрак чиқиши, юрак қисқаришининг кўпайиши ва қон томир қаршилиги билан бирга бўлган қоннинг умумий ҳажмидир. Ўз навбатида, гипердинамик синдром каби жигар циррозининг қайтариб бўлмайдиган оқибатлари ва асоратларига олиб келади. Баъзи кардиоваскуляр ўзгаришлар жигар трансплантациясидан кейин қайтарилиши мумкин ва шу билан портал гипертензиянинг патофизиологик аҳамиятини кўрсатади. Ушбу шарҳда биз патофизиология ва гипердинамик синдромнинг кўринишларини кўриб чиқдик, чунки улар маълум ва тўғридан-тўғри портал гипертензия билан боғлиқ (варикоз томирлари, ассит, жигар энцефалопатияси ва гепаторенал синдром), шунингдек, камдан-кам учрайдиган ва кейинги ўрганишни талаб қиладиган (портопулмонал гипертензия, сирротик кардиомиопатия).
