ВЗАИМОСВЯЗЬ ПОЛИМОРФИЗМА rs2476601 ГЕНА PTPN22 С ПРОВОСПАЛИТЕЛЬНЫМ ЦИТОКИНОВЫМ ПРОФИЛЕМ, АКТИВНОСТЬЮ И СТРУКТУРНЫМ ПОВРЕЖДЕНИЕМ СУСТАВОВ ПРИ РЕВМАТОИДНОМ АРТРИТЕ (67-72)

Abstract
Цель. Изучить связь полиморфизма rs2476601 (C1858T) гена PTPN22 с провоспалительными цитокина ми ФНО-α, ИЛ-6 и ИЛ-17, клиническими индексами активности, функциональными ограничениями и рентге нологическим повреждением суставов у больных ревматоидным артритом (РА) различных стадий. Материалы и методы. В исследование были включены 200 больных РА, соответствующих критериям ACR/EULAR 2010, и 30 практически здоровых лиц. Пациенты распределены на группы ранней (n=60), развёрнутой (n=80) и позд ней (n=60) стадий. Активность оценивали по DAS28-СРБ, SDAI и CDAI, функциональный статус - по HAQ DI. Сывороточные уровни ФНО-α, ИЛ-6 и ИЛ-17 определяли методом ИФА. Генотипирование rs2476601 вы полняли секвенированием фрагмента PTPN22. Заключение. Гомозиготный вариантный генотип TT rs2476601 PTPN22 характеризует провоспалительный и структурно-деструктивный фенотип РА. Его высокая вариабель ность среди больных с развёрнутой и поздней стадиями сочетается с повышением ФНО-α, ИЛ-6 и ИЛ-17, вы сокой активностью и выраженным рентгенологическим повреждением суставов.
