HISTORY OF THE DEPARTMENT


  The department of hospital pediatrics No. 2 of SamMI was organized on September 22 - September 1981 by order No. 105 - 0 of the rector of SamMI, prof. S.A. Aripova. The head of the department was appointed a young doctor of honey. J. Ishkabulov. On the eve of the organization of the department, he defended his doctoral dissertation with the advice of a full member of the Medical Academy of Sciences (AMS) of the former Union of Soviet Socialist Republics (USSR), Professor V.A. Tabolin on the topic: "Kidney disease in children in the hot climate of Uzbekistan." Already in October (01.10. 1981) the department was visited by the teacher of the head of the newly organized department, academician V.A. Tabolin. Academician V.A. Tabolin, on the basis of the new department - in the city children's clinical hospital, consulted patients with the participation of the department staff, hospital residents and students.

  In a conversation with the staff of the department V.A. Tabolin recommended "Acquired and hereditary kidney diseases in children" as a scientific direction, emphasizing the importance of studying the hereditary aspects of the pathology of the urinary system (OMS) in the Uzbek population, taking into account the high inbreeding coefficient (frequency of consanguineous marriages) and especially noted the relevance of studying dysmetabolic nephropathies, taking into account endemicity of the region for urolithiasis (Urolithiasis). He advised to start scientific research by studying the age and sex characteristics of the spread (epidemiology of CHI diseases) and their nosological structure.

  The staff of the department under the leadership of Dzhanhuraz Ishkabulovich from the first days of education worked in close contact with the Department of Hospital Pediatrics No. 2 MOLGMI (Head Prof. V.A. Tabolin), Department of Nephrology (Head of Prof. V.I. Naumova), Research Institute of Pediatrics USSR (Director Academician M. Ya. Studenikin), Department of Nephrology (Head of Prof. MS Ignatova). Moscow Research Institute of Pediatrics and Pediatric Surgery (director Academician Yu.E. Veltischev). Thanks to these large scientific nephrological schools of the former Soviet Union, a number of modern methods for studying the metabolic status of the subjects were established at the department in a short time: chromatography of free amino acids in blood and urine by high-voltage electrophoresis, chromatography of tryptophan metabolites (T.F. Ernazarov), determination of uric acid, oxalates (T.K. Knapp), blood lipidogram by thin-layer chromatography on Solifoli plates with subsequent densitometry (A.A. Akhmatov, N.A. Karimova, R.R. Tukhvatulina), column chromatography of connective tissue metabolites (K.R. Dilmuradova) and others. Methods for assessing the clinical and diagnostic significance of fermenturia (neutral and acidic α - glucosidase, β - hexosaminidase, β - galactosidase) in kidney disease in children were established (AS Diveeva). Hormonal research has been established in cooperation with the radioimmunological laboratory of the Department of Radiation Diagnostics and Radiation Therapy of SamMI (Head of the MD, Prof. TM Mamadzhanov).

  As a result of studies conducted over several years, for the first time in Uzbekistan, the epidemiology of kidney diseases in the Zerafshan Valley (Samarkand city) was studied in 10 pediatric sites of children's polyclinics No. 2, 4, 5

  The research results showed a significant prevalence of CHI pathology - 7.4% (74 ± 2.66 per 1000 examined). Studies carried out in Russia in the 70s and 80s of the twentieth century revealed the pathology of compulsory health insurance in 29 out of 1000 examined.


  According to the results of scientific research, it was established that (in the 80s of the twentieth century) among 715 patients with nephropathies, the nosological structure was as follows: urinary tract infections, including pyelonephritis (49.2%), followed by dysmetabolic nephropathies (29.1% ), and glomerulonephritis 20%. For the first time in the climatic conditions of Uzbekistan, the frequency of urate nephropathy in the structure of nephropathy was established. The clinical and biochemical characteristics of urate nephropathies are presented. The advantages of conducting medical examination on a family basis in order to identify pre-manifest forms of pathology and increase the effectiveness of preventive measures are substantiated.


  A complex of clinical and laboratory criteria for the diagnosis of urate nephropathies in the pre-manifest stage (pedigree analysis, indicators of uricemia and uricosuria, fractional clearance of uric acid, xanthine oxidase activity and assessment of ammonio-acidogenetic renal function) has been developed and proposed for clinical practice. In 1997, the first independent monograph in the CIS countries, "Dysmetabolic nephropathies in children," was published in the "Library of the Practitioner" series (J. Ishkabulov, S. Abdurakhmanova, 1997), in which, along with an analysis of world literature data, almost 20 years research results of the staff of the Department of Hospital Pediatrics No. 2 on dysmetabolic nephropathy in children. For the first time, the monograph presented the results of segregation analysis using various genetic hypotheses (monogenic, multifactorial) and substantiated that DZMN correspond to a polygenic (multifactorial) model of pathology. To facilitate the perception by practitioners, many detailed clinical examples of their own (nephropathy against the background of uraturia, hyperoxaluria, cystinuria) were given, fragments of the pedigrees of these patients were given. For the first time, dietary-drug therapy was given, differentiated depending on the type of dysmetabolism, therapy, especially depending on the stage of the evolution of the disease (pre-manifest stage, DZMN, interstitial nephritis, secondary pyelonephritis against this background, urolithiasis). Metabolic aspects of ICD in children in Uzbekistan were specially highlighted.

  Under the leadership of J.I. Ishkabulov, 3 doctoral and 13 master's theses were defended. Currently, his students work in almost all CIS countries. Being the founder of pediatric nephrology in the Republic of Uzbekistan, Dzhanhuraz Ishkabulovich has published more than 400 printed works: 2 of them are textbooks, 2 monographs, more than 40 teaching aids.

  From June 2012 to October 2019, the department was headed by Candidate of Medical Sciences, Associate Professor B.A. Yuldashev.

  Since October 2, 2019, the department is headed by Doctor of Medical Sciences, Associate Professor Akhmedzhanova N.I. Under her supervision, 1 doctoral and 2 master's theses are carried out on topical issues of modern pediatrics.


  Currently, the Department of Pediatrics No. 2 of SamMI is located at the clinical base of the Samarkand Regional Children's Multidisciplinary Medical Center, where students study in specialized departments: pulmonology, allergology, cardiology, nephrology, gastroenterology, hematology, department for young children, functional, laboratory diagnostics and admissions branches.

SPIRITUAL EDUCATIONAL WORK


  Caring for the younger generation, striving to bring up a healthy, harmoniously developed person is one of the features of the national character of the Uzbek people. Under the guidance of curators, students of the faculty actively participate in public events, organize cultural leisure, visit historical monuments and sights of Samarkand, theaters and museums.

  Much attention is paid to physical education and sports. Students attend sports sections in swimming, tennis, chess, football, basketball, volleyball, national wrestling, martial arts, athletics, which function on the basis of the sports complex of the institute. They actively participate in both regional and Republican sports competitions.

  The slogan of our department is "High spirituality is an invincible force." The book of the first President I.A. Karimov under this title is considered the main guide for every employee and student of the faculty. As the Eastern wisdom says - "It is not possible to separate education from upbringing and vice versa, upbringing from education."



  The department conducts work among students and faculty:

  • with the aim of increasing and improving the spiritual and worldview qualities of the individual and universal and national principles on the basis of the works and speeches of President Sh.M. Mirziyoyev, decrees and decisions of the Oliy Majlis and the Cabinet of Ministers, as well as local governing bodies concerning the concept of upbringing a harmoniously developed generation, especially in the spiritual and educational aspect;
  • to promote creative work in the republic in the process of successful implementation of reforms;
  • in the formation of a philosophy of a healthy lifestyle among students;
  • to improve the political and legal culture among students;
  • on timely delivery to students of information about social, political and creative events taking place in the world and in the country;
  • on the formation of a healthy spiritual atmosphere among students;
  • on the organization of students' leisure, the organization of various hobby groups, corresponding to the direction and specialization of the educational institution.

  Each curator has a work plan, on the basis of which he conducts various activities with the curator group. Each curator has information about each student, and his daily routine. There are also albums and discs that reflect the plots of the events. The dean and deputy deans of the faculty regularly meet with the group curators and students. Every Thursday, classes begin with an introduction to the Information Hour.


  Spiritual and educational work is carried out on the basis of an approved plan, according to which various events are held dedicated to historical dates, national holidays, sports competitions, etc. Various meetings are held together with internal affairs bodies, law enforcement agencies, centers of spirituality and other partner organizations.

EDUCATIONAL-METHODOLOGICAL AND SCIENTIFIC WORK


  During this period of the department's existence, the staff published in print more than 600 scientific works, including more than 150 journal articles, in the republican and foreign press. More than 30 rationalization proposals were made to improve the methods of diagnosis, treatment, rehabilitation and primary prevention of CHI diseases in children, 3 patents for inventions were received. Based on the results of our own research, 10 guidelines for practitioners, 30 educational guidelines for senior students of the pediatric faculty were prepared.

  1 popular scientific edition for a wide range of readers, 2 monographs on pediatric nephrology, 4 textbooks for students in the state language and 1 textbook have been published. 13 Ph.D. and 3 doctoral dissertations were defended. It should be noted that the team of the department has a priority in the study of epidemiology, nosological structure of CHI diseases, various aspects of nephropathies of metabolic genesis, preventive prevention of dysmetabolic nephropathies, neonatal nephrology in climatic and ecological conditions of Uzbekistan and in the Uzbek population.

  In 1991, to the 10th anniversary of the department, the first attempt was made to generalize the materials obtained over 10 years in a popular scientific publication in the state language: "Bolalarda buyrak casalliklarida parhez", in which they tried to sharpen the attention of the medical community and the population to the importance of the problem of dysmetabolic nephropathies (DZMN) in our region (Zh. Eshkobilov, K. Dilmuradova, 1991), recommendations are given on diet - prevention and diet therapy of DZMN, taking into account the peculiarities of the national cuisine.

  In 1997, the first in the CIS countries independent monograph "Dysmetabolic nephropathies in children" (J. Ishkabulov, S. Abdurakhmanova, 1997) was published in the "Library of a Practitioner" series, in which, along with the analysis of world literature data, almost 20 years research results of the staff of the Department of Hospital Pediatrics No. 2 on dysmetabolic nephropathies in children.

  In 2001, a two-part textbook in the state language "Bolalar Kasalliklari" for senior students of the pediatric faculty of medical institutes was published, compiled by many years of work of the department staff. In the section "Nephrology" of this manual, the results of many years of scientific research of the Department of Hospital Pediatrics No. 2 of SamMI, on issues of nephrology of childhood, were widely used.

  In 2008, a monograph in the state language "Bolalar Nephrourology" was published (DI Ishkabulov, YM Akhmedov with the participation of the department staff), where, along with an analysis of the world literature on this problem, their own clinical, genealogical, laboratory data on nephro-urological pathology of the department staff.

  Currently, continuing the main direction of the department, the staff conduct their scientific research on the most topical issues of pediatric nephrology. The work is carried out using the latest methods of laboratory diagnostics and digital medicine equipment. A modern method for the diagnosis of renal pathology based on the assessment of the level of cystatin - C in the blood plasma is being introduced and studied. Another of the latest priority areas of research work is the diagnosis of cardiovascular complications in acute and chronic kidney pathology. In particular:
