Геморрой является одним из наиболее распространённых аноректальных заболеваний в клинической практике с глобальной распространённостью от 4,4% до 36%. Хирургическое лечение геморроя III–IV степени традиционно основывалось на геморроидэктомии по Миллигану–Моргану (ГММ), признанной «золотым стандартом» вмешательства. В 1998 году Антонио Лонго предложил альтернативный метод — степлерную геморроидопексию (СГП, или процедуру PPH), обеспечивающую значительно менее выраженный послеоперационный болевой синдром и более быстрое восстановление. Настоящий систематический обзор сравнивает долгосрочные анатомические и функциональные результаты обоих методов на основании данных рандомизированных контролируемых исследований, метаанализов и проспективных когортных исследований. Совокупность данных свидетельствует о чётко выраженном компромиссе: СГП обеспечивает преимущества в краткосрочном периоде — меньшую боль, сокращение времени операции и ускоренное восстановление трудоспособности, — однако сопряжена с достоверно более высокой частотой долгосрочных рецидивов и повторных вмешательств, особенно при геморрое IV степени. ГММ сохраняет превосходство по долгосрочной радикальности при запущенных формах заболевания. Результаты обзора позволяют сформулировать дифференцированный алгоритм выбора метода с учётом степени геморроя и предпочтений пациента.
