ВОССТАНОВЛЕНИЕ МЕНИСКА У СПОРТСМЕНОВ: ФУНКЦИОНАЛЬНЫЕ ИСХОДЫ И ВОЗВРАТ В СПОРТ (20-26)

Аннотация

Артроскопическое восстановление мениска - предпочтительный метод лечения разрывов мениска у спортсменов, однако возврат в спорт (ВВС) после операции остаётся существенной проблемой. Цель - оценить факторы, влияющие на заживление, функциональные исходы и ВВС после изолированного восстановления мениска. Проведено ретроспективное исследование 53 спортсменов моложе 50 лет без сопутствующего повреждения передней крестообразной связки (ПКС) и с наблюдением не менее 12 месяцев; оценивались субъективные шкалы IKDC и Tegner. При среднем сроке наблюдения 21 ± 8,5 месяца субъективный балл IKDC улучшился с 48,5 до 76,2; частота ВВС составила 86,8%, при этом 66% когорты вернулись к дотравматическому уровню; среднее время возврата к бегу и тренировкам - 6,1 ± 2,8 месяца. С положительными исходами ассоциировались операция в течение трёх месяцев от травмы, возраст менее 20 лет, статус некурящего, вертикальный тип разрыва и высокий дооперационный функциональный статус. Восстановление мениска обеспечивает хорошие кратко- и среднесрочные исходы и высокую частоту ВВС; необходима индивидуализированная оценка для оптимизации восстановления и формирования реалистичного прогноза.
