Аннотация
Aim: to describe the MRI features and radiologic–pathologic correlation of a rare paediatric cortical tumour, pleomorphic xanthoastrocytoma (PXA), using a clinical case. Materials and methods: a 4-year-old girl present-ing with headache, nausea, vomiting, dizziness and gait instability underwent 3.0 T MRI (DWI/ADC, SWI, con-trast-enhanced T1, diffusion tensor tractography), microsurgical resection, and histological and immunohisto-chemical examination. Results: MRI showed a 5.8×5.7×6.4 cm predominantly cystic cortico-subcortical mass in the lateral right frontoparietal region with a peripheral solid component, irregular peripheral enhancement, marked vasogenic oedema and mass effect. Subtotal resection was performed. The tumour was positive for GFAP, Olig2 and CD34 and negative for IDH1 and BRAF V600E, with a Ki-67 index of approximately 35%; PXA, CNS WHO grade 3 (ICD-O 9424/3), was diagnosed, and adjuvant radiotherapy was given. Conclusion: PXA should be included in the differential diagnosis of large superficial cystic cerebral masses in children; the final diagnosis requires interpretation of MRI findings together with histological and immunohistochemical data.
