СОВРЕМЕННЫЕ ПРИНЦИПЫ ВЫБОРА ХИРУРГИЧЕСКОЙ ТАКТИКИ ПРИ КИСТОЗНОМ ЭХИНОКОККОЗЕ ПЕЧЕНИ: ОТ СТАДИИ КИСТЫ К ПЕРСОНАЛИЗИРОВАННОМУ ЛЕЧЕНИЮ (447-456)

Аннотация
Кистозный эхинококкоз печени сохраняет высокую медицинскую и социально-экономическую значи-мость, особенно в эндемичных регионах Централь-ной Азии. Расширение диагностических и хирургиче-ских возможностей привело к формированию широ-кого спектра лечебных подходов - от динамического наблюдения и противопаразитарной терапии до PAIR, катетеризационных методов, органосохраня-ющих операций, тотальной перицистэктомии и различных вариантов резекции печени. Цель обзора - систематизировать современные данные о крите-риях выбора метода лечения кистозного эхинокок-коза печени и определить факторы, которые долж-ны учитываться при персонализации хирургической тактики. Анализ литературы показывает, что классификация WHO-IWGE является базовым ин-струментом стадийно-ориентированного лечения, однако сама по себе не отражает размер, множе-ственность и топографо-анатомическую слож-ность кист, их отношение к магистральным сосу-дам и желчным протокам, а также функциональ-ный резерв печени. Современные данные в целом ука-зывают на меньший риск рецидива после радикаль-ного удаления паразитарного очага, однако увеличе-ние объема резекции не должно рассматриваться как самоцель. Наиболее рациональна стратегия, обеспечивающая максимальную паразитарную ра-дикальность при минимально необходимой потере функционирующей паренхимы и приемлемом опера-ционном риске. Перспективным направлением явля-ется разработка интегральных алгоритмов, объ-единяющих стадию WHO-IWGE, морфологию и раз-меры кисты, ее топографию, наличие цистобилиар-ной коммуникации, множественность поражения, соматический статус пациента и технические воз-можности хирургического центра.
