Волчаночный нефрит: эпидемиология прогрессирования и современные стратегии доказательной терапии (47-50)

Аннотация

Волчаночный нефрит (ВН) остается одним из наиболее тяжелых и прогностически неблагоприятных проявлений системной красной волчанки (СКВ). В данном обширном литературном обзоре, основанном на синтезе современных данных, проанализированы паттерны прогрессирования ВН в терминальную хроническую болезнь почек (ТХБП). Рассматриваются эпидемиологические факторы риска, частота гистологической трансформации классов нефрита, а также предикторы неблагоприятного исхода с особым акцентом на особенности течения в терапевтической практике и развитие кардиоренальных осложнений. Детально освещена эволюция доказательной базы лечения: от стандартных схем индукционной и поддерживающей иммуносупрессии до инновационных таргетных и биологических препаратов (белимумаб, воклоспорин, анти-CD20 агенты). Проанализированы текущие вызовы клинической практики, включая необходимость минимизации глюкокортикостероидов и поиск надежных неинвазивных биомаркеров.
