Annotatsiya

Оксидативный стресс (ОС) является ключевым патогенетическим механизмом развития почечной дис функции у пациентов со стенокардией, особенно в периоперационный период аортокоронарного шунтирова ния (АКШ). Избыточная генерация реактивных форм кислорода приводит к эндотелиальному повреждению, тубулярной гипоксии, нарушению микроциркуляции и активации провоспалительных цитокинов, что способ ствует формированию острого повреждения почек. В обзоре представлены современные данные о молекуляр ных звеньях ОС, его роли в ишемии-реперфузии, сравнительных особенностях on-pump и off-pump АКШ, а также диагностической значимости маркеров (МДА, СОД, цистатин С, гомоцистеин). Показано, что оценка маркеров ОС обладает высоким прогностическим потенциалом в раннем выявлении почечной дисфункции и выборе нефропротективных стратегий.
